Working With the Daily COVID-19 Health Check Questionnaire

Complete the Daily COVID-19 Health Check for all persons assigned to your Health Portal account.

Depending on how your district has set up the Daily Health Check, you may be presented with prescreening questions that determine whether the Daily Health Check Questionnaire is required for the day. 

You can access the Health Portal in a web browser on your computer (desktop) or from the Frontline Health Portal mobile app.

 - Mobile App: If you want to access the Health Portal from your mobile device, you can download the Frontline Health Portal Mobile app. Go to the Apple App Store or Android Apps on Google Play and search for “Frontline Health Portal” to find the Frontline Health Portal mobile app.

Completing the Questionnaire in the Desktop App

Daily Health Check(s) For Banner

Use the blue "Daily Health Check(s) For" banner at the top of the Health Portal screen banner to navigate to the Daily Health Check Questionnaire for each person assigned to your Health Portal account.

If the Daily Health Check process has not been completed for a person, "Start" is displayed. Click anywhere in the person's row to start the process. The screen that is displayed next depends on how your district has set up the Daily Health Check.

  • If your district uses prescreening questions, the Daily Health Check Prescreen opens with the question, "1. Will [Person Name] be attending a [Your District] facility or event today?" Proceed to Prescreening Questions.
  • If your district does not use prescreening questions, the Daily Health Check Questionnaire opens. Proceed to Daily Health Check Questionnaire.

Information in the Health Banner

As you complete the Daily Health Check process for each person assigned  to your Health Portal account, the status of each person's health check is displayed in the Daily Health Check(s) For banner, as described below.

Status Description
Start   The Daily Health Check process has not been completed for the person. Click to go to either Prescreening Questions or the Daily Health Check Questionnaire, depending on how your district has set up the Daily Health Check.
  Cleared The Daily Health Check process has been completed, and the person has passed the Daily Health Check Questionnaire. 
Not Cleared The Daily Health Check process has been completed, and the person has not passed the Daily Health Check Questionnaire. This indicator has a red background. 
  Not Required The Daily Health Check process has been completed, and in the Prescreening Questions, you acknowledged that the person will be either absent or participating remotely and would not be at a district facility or event today. Therefore, the Daily Health Check Questionnaire is not required. (Your district controls whether Prescreening Questions are used.) 
 Note: You can also access the Daily Health Check questionnaire for a person from the "To Do List" by clicking the   Form icon on the row for the Daily COVID-19 Health Check to open the questionnaire. You can use the Select Person field to view another the information of another person who is assigned to you.

Prescreening Questions

If your district uses prescreening questions with the Daily COVID-19 Health Check, you can indicate whether the selected person will be attending a district facility or event in-person.

  • For in-person attendance, you will select the district location(s) and then proceed to the Daily Health Check Questionnaire.
  • If the person will not be physically attending a district facility or event, you will indicate whether the person is learning remotely or absent. You will not need to complete the Daily Health Check Questionnaire for this person. 

The Daily Health Check Prescreen is shown below. For the first question, "1. Will [Person Name] be attending a [Your District] facility or event today?",  select either "Yes" or "No" as appropriate.

How you proceed depends on your answer to the first question. 

If you answered "Yes" for Question 1:

  • The "2. Which [Your District] facility or event will [Person Name] be attending today?" field opens, as shown below.

Question 2 if you select "Yes" for Question 1
  • For Question 2, the primary location associated with the person is selected by default, but if you need to change this or select additional locations, click to open a selection list of district locations. Click to add or remove locations as necessary. 
  • Click Next to proceed to the Daily Health Check Questionnaire.

If you answered "No" for Question 1:

  • The "2. [Person] will not be attending a [Your District] facility or event today because [Person] is" field opens, as shown below.

Question 2 if you select "No" for Question 1
  • For Question 2, select the appropriate response for why the person will not be attending a facility or event.
  • Click Next to display the Health Check Not Required Acknowledgment.

  • Read the statement and any additional instructions your district may have provided.
  • To proceed with acknowledging the information, select "Yes" in the Please acknowledge field.
  • Click Next to display the Daily Health Check notice with the message "Health Check Not Required".

  • The Daily Health Check process is complete for this person, and in the Daily Health Check(s) For banner, the status of  " Not Required" is displayed.
Refer to  Buttons in the Daily Health Check Notice  for information about actions you can take in the Health Check Not Required notice.

Daily Health Check Questionnaire

If your district does not use prescreening questions, the Daily Health check is displayed when you click a row with the status of "Start  " in Daily Health Check(s) For banner.

If your district uses Prescreening Questions, you must complete the Daily Health Check Questionnaire for the person if you answered "Yes" to the question, "1. Will [Person Name] be attending a [Your District] facility or event today?"

Daily COVID-19 Health Check Questionnaire 

Complete the questionnaire for the selected person, and click Submit once you are finished. A Daily Health Check notice is displayed that shows whether or not the person passed the questionnaire. 

 Note: The questions displayed on the Daily COVID-19 Health Check Questionnaire are determined by your district.

When you complete the Daily Health Check process for a person, their Daily Health Check Notice is displayed.

For a completed Daily Health Check, you will see one of the messages/statuses below.

Buttons in the Daily Health Check Notice

In the Daily Health Check Notice, you can use buttons to take additional actions.

You have the following button options:

Button Action
Close Click Close to close the notice. You are returned to the Home screen where you can complete any outstanding items on the To Do list. Once you close the notice, you cannot access it again until the next time you take the questionnaire.
Retake If you want to clear your selections in the questionnaire and resubmit it, click Retake. Your previous selections are cleared, and you may retake the questionnaire.
Print To print the notice with your status result, click Print
Email To email the notice with your status result, click Email to open the Share Email box. In the Email field, enter the email address to which you want to send the notice and click the Send Email button.
Be sure to complete the Daily COVID-19 Health Check Questionnaire for each person listed in the Daily Health Checks banner. Click Start  to open and complete the questionnaire for that person.

Deleting the Current Day's Health Check Questionnaire

If you have closed the Daily Health Check notice and would like to retake the questionnaire for the current day, you can delete it. To do this, select the History tab, then click Delete in the row with the questionnaire. The Confirm Delete box opens with the message, "Are you sure you want to delete the Questionnaire Form for today?" Click the OK button to proceed with the deletion.

Completing the Questionnaire in the Mobile App

My People Screen

If you have multiple people assigned to your account, the My People screen is displayed when you first log in to the Health Portal Mobile app. From the My People screen, you can navigate to the Daily Health Check Questionnaire for each person assigned to your account.

If you have only one person assigned to your account, the To Do List is the first screen displayed when you log in to the Health Portal Mobile app.

If the Daily Health Check process has not been completed for the person for the day, "Health Check Due" is displayed under the person's name. To start the Daily Health Check, tap the person's name to go to the To Do List.

Information on the My People Screen

As you complete the Daily Health Check process for each person assigned to your Health Portal account, the status of each person's health check is displayed on the My People screen, as described below.

Status Description
Health Check Due The Daily Health Check process has not been completed for the person. Tap the person's name to go to the To Do List.
 Cleared The Daily Health Check process has been completed, and the person has passed the Daily Health Check Questionnaire. 
Not Cleared The Daily Health Check process has been completed, and the person has not passed the Daily Health Check Questionnaire. This indicator has a red background. 
 Not Required The Daily Health Check process has been completed, and in the Prescreening Questions, you acknowledged that the person will be either absent or participating remotely and would not be at a district facility or event today. Therefore, the Daily Health Check Questionnaire is not required. (Your district controls whether Prescreening Questions are used.) 

To Do List

On the To Do List, tap Daily COVID-19 Health Check to start the Daily COVID-19 Health Check process. 

 Note: If you have multiple people assigned to your account, the To Do List is displayed when you tap a person's name on the My People screen. You can also access the To Do list by tapping the menu icon (HealthPortalMenu.png) , then tapping To-Do / History in side navigation.

The screen that is displayed next depends on how your district has set up the Daily Health Check.

  • If your district uses prescreening questions, the Daily Health Check Prescreen opens with the question, "1. Will [Person Name] be attending a [Your District] facility or event today?" Proceed to Prescreening Questions.
  • If your district does not use prescreening questions, the Daily Health Check Questionnaire opens. Proceed to Daily Health Check Questionnaire.

Prescreening Questions

If your district uses prescreening questions with the Daily COVID-19 Health Check, you can indicate whether the selected person will be attending a district facility or event in-person.

  • For in-person attendance, you will select the district location(s) and then proceed to the Daily Health Check Questionnaire.
  • If the person will not be physically attending a district facility or event, you will indicate whether the person is learning remotely or absent. You will not need to complete the Daily Health Check Questionnaire for this person.

The Daily Health Check Prescreen is shown below. For the first question, "1. Will [Person Name] be attending a [Your District] facility or event today?",  tap Select Yes/No to open the Select Item box, then tap to select either "Yes" or "No" as appropriate, and tap Ok to close the box.

  • Tap Next to proceed to the Daily Health Check Questionnaire.

If you answered "No" for Question 1:

  • The "2. [Person] will not be attending a [Your District] facility or event today because [Person] is" field opens, as shown below.

  • For Question 2, tap Select a reason to open the Select Item box, then tap to select the appropriate reason, and tap Ok to close the box.
  • Tap Next to display the Health Check Not Required Acknowledgment.

  • Read the statement and any additional instructions your district may have provided.
  • To proceed with acknowledging the information, tap Please acknowledge to open the Select Item box. "Yes" is selected by default. Tap Ok.
  • Click Next to display the Daily Health Check notice with the message "Health Check Not Required".

  • The Daily Health Check process is complete for this person, and on the My People screen, the status of  "  Not Required" is displayed.
Refer to Options on the Daily Health Check Notice for information about actions you can take in the Health Check Not Required notice.

Daily Health Check Questionnaire

If your district does not use prescreening questions, the Daily Health check is displayed when you tap Daily COVID-19 Health Check on the To Do List. 

If your district uses Prescreening Questions, you must complete the Daily Health Check Questionnaire for the person if you answered "Yes" to the question, "1. Will [Person Name] be attending a [Your District] facility or event today?"

Complete the questionnaire for the selected person, and tap Submit once you are finished. A Daily Health Check notice is displayed that shows whether or not the person passed the questionnaire.

Options on the Daily Health Check Notice

You have the following options on the Daily Health Check Notice screen:

  • To email or print the Daily Health Check notice, tap Share Result and navigate to the desired functionality.
  • For a "Health Check Not Cleared" or "Health Check Not Required" notice, tap Next to view any specific instructions your district has configured. 
  • Tap  Back to return to the To Do List where you can complete any outstanding items on the To Do list. Once you navigate from the Daily Health Check notice, you cannot access it again until the next time you take the questionnaire.

Deleting the Current Day's Health Check Questionnaire

If you have closed the Daily Health Check notice and would like to retake the questionnaire for the current day, you can delete it on the To Do List.

To do this, tap Delete Today's Health Check on the To Do List/History screen.

Completing Another Daily COVID-19 Health Check Questionnaire

Be sure to complete the Daily COVID-19 Health Check Questionnaire for each person assigned to your Health Portal account.

Tap the menu icon (HealthPortalMenu.png) , then tap My People in side navigation to view your list.  Complete the Daily COVID-19 Health Check process for any person with the "Health Check Due" message.
